TAWREF is currently seeking to recruit a competent, dynamic, energetic, committed, experienced, and qualified Tanzanian to work on the Epidemic Control (EpiC) Project.
Position: Case Manager
Position Description
The Case Manager will collaborate with Community Engagement Officers and Biomedical Officers to implement an HIV prevention program targeting populations at risk in the region.
The role involves coordinating and monitoring the linkage of newly diagnosed HIV-positive individuals to Care and Treatment Centres (CTCs). The Case Manager will supervise all activities related to retention in care, including tracking clients who miss appointments or are lost to follow-up, and ensuring they return to care.
The Case Manager will also ensure that all navigation processes follow established care and treatment standards and that a client-centered approach is maintained during service delivery.
Additionally, the Case Manager will supervise and build the capacity of Peer Navigators through monthly progress review meetings and coordinate case management activities between health facilities and community structures to ensure a continuum of care.
The position reports to the CSO Program Manager and works closely with the Regional Biomedical Technical Advisor.
Key Responsibilities
- Liaise with the District Community-Based HIV Coordinator (DCBHCO).
- Coordinate with health facilities and Peer Navigators for client enrollment from the community.
- Support retention of clients on Antiretroviral Therapy (ART) and provide feedback to health facilities.
- Manage client appointments and ART refills.
- Track missed appointments and coordinate follow-up.
- Support community-based ART services by managing refill appointments.
- Track lost-to-follow-up clients and facilitate their return to care.
- Provide mentorship to Peer Navigators.
- Serve as a link between Peer Navigators and health facilities.
- Supervise peer navigation teams.
- Track and report completed and pending referrals.
- Conduct daily follow-ups on case management activities with the Biomedical Field Officer.
- Reconcile and compile case data into regional tracking registers and outcome registers weekly.
- Follow up on open cases to ensure beneficiaries receive outcomes.
- Conduct site visits (at least 70% field work).
- Collaborate with the Data Manager / M&E Officer to report referral and tracking data to the MER team.
- Prepare weekly and monthly reports on tracking services and partner notifications, including success stories.
- Participate in CSO meetings and CHMT meetings.
- Document best practices for linkage for Key and Vulnerable Populations (KVP).
- Perform any other duties assigned by the line manager.
Qualifications and Experience
- Bachelor’s Degree or Diploma in:
- Clinical Management
- Nursing
- Social Work
- Counselling
- Psychology
- At least 3 years of experience in case management for community-based HIV services.
- Experience working with both literate and non-literate audiences, including participatory approaches.
- Ability to handle data with high integrity.
- Experience in Key Population (KP) programs is an added advantage.
- Experience supervising field teams.
Required Skills
- Good communication skills in Kiswahili and English.
- Ability to work effectively in a team.
- Basic computer skills, including:
- Microsoft Excel
- Microsoft Word
Other Information
- Contract Duration: 3 months (with possibility of extension).
